It has come to my attention that the backup supply of one of our critical reagents was used, and not only the log was not filled out, but also the lab manager was not made aware of that fact. The lack of reagent caused us to get behind on production and have to have the reagent shipped to the lab overnight for an extra amount of money. This type of situation, which causes the lab to lose time and money, is completely preventable. In order to not have such a scenario happen again, I would to like to establish or re-inforce several rules.

First, it is mandatory to fill out a log anytime supply of a reagent is used, no matter what type of reagent it is, and especially if it is a critical reagent. The log should be properly filled out with the date, the tech’s name and the type of reagent that was taken out of the supply room.

Secondly, only the lab manager or the designated lab technician/leader should have access to the backup supply of critical reagents. They will be the only ones allowed to access that supply, and no other person in the lab should be permitted to access it, unless they have been given permission.
